The development of Chronic Disease Management in the Asia-Pacific Region has been very rapid since the first Singapore Disease Management Conference in May 2001. Many disease management initiatives have proliferated over the last 8 years to improve the quality and efficiency of care for patients with chronic illnesses.

This year, the National Healthcare Group (NHG) Disease Management Department is partnering with the Primary Care Academy (PCA) to co-organise
The Singapore Disease Management and Primary Care Forum 2009. This forum is held in conjunction with the NHG Annual Scientific Congress 2009.

The forum's theme is
'Person-centered Care: Making It a Reality', chosen to emphasise the principal role of patients in the healing relationship with their care-providers and how healthcare providers should align their system to suit their patients' needs. We have invited a panel of experts from the UK, USA and Asia, including Professor David Colin-Thome, Dr Alan Cohen, Mr Brian Austin, Professor Ken Resnicow and many others.

The themes for the symposia range from disruptive innovation and building blocks for primary health and chronic care to the involvement of allied health professionals in patient care and patient-centered medical homes.

The "5-minute Office Procedures in Busy GP Clinics" is a practical workshop for Family Physicians to handle
"triggers, sprains and strain", "wrinkles, sags and bulges" and
"needling and kneading away pain" in their clinics.

This forum promises to be enriching for physicians and nurses, case and care managers, paramedical professionals, medical social workers, healthcare managers and administrators working in the Asia-Pacific Region. You will be able to network with peers and leaders in disease management.
