INSTRUCTIONS TO PRESENTING AUTHOR
Please read before submitting your abstracts.
 
1) GENERAL INFORMATION
Participants are invited to submit abstracts for topics relating to Chronic Disease Management

Disease Control, Health Promotion & Health Prevention
Disease Management Programmes in Action
Evidenced-Based Medicine
Health Information Technology

All abstracts must be submitted through the Online Submission Form. Abstracts submission via mail, fax or email will not be accepted.
Submission will be opened from 15 November 2007.

Abstracts must be submitted according to the instructions below and must be received by 31 January 2008. No abstract will be considered after the closing dateline.
2) SUBMISSION INSTRUCTIONS
Abstracts must be typed in English on the Abstract Submission Form.

Title: Use a concise title (5 or 6 words, if possible) that reflects abstract content. Capitalise the first letter of first word and all other words except prepositions, conjunctions, and articles. Do not use acronyms, abbreviations, and initials in a title.
Abstracts: All abstracts must be typed in Times New Roman font size 10 with single spacing, and must fit within the box.
Text length (not including title and author information) must be at least 100 words and not exceed 250 words.
The purpose of the study, methodology, results and conclusion should be clearly stated.
References should be avoided.

Abbreviations should be used only for common terms. For uncommon terms, the abbreviation should be given in brackets after the first full use of the word.
Do not use tables or graphics in the abstracts. All data must be in narrative form.
3) REVIEW, ACCEPTANCE & NOTIFICATION
Receipt of abstract submission will be acknowledged via email.

The Scientific Committee will review all abstract submissions. Evaluation by the Committee will be based on quality, clarity and relevance for our audience, as well as the significance of the material.

Shortlisted authors for the Poster Presentation and Oral Presentation awards will receive notification via email of abstracts' acceptance in February 2008.

Presenting authors will be informed via email the details of presentation (including date and time) and between March - April 2008.

Submission of abstracts therefore constitutes the authors' consent to have the abstracts published in the Singapore Disease Management 2008 Abstracts Book. Abstracts may also be edited for publication.
4) POSTER PRESENTATION

Poster Presentation: Authors of poster presentations are provided with an approximately 95 cm (wide) x 200 cm (high) poster board to display the poster.
Exact poster specifications and sessions will be included in the notification of acceptance.

Poster presenters are requested to stand-by their posters during the poster sessions and also to take note the poster set-up and take-down schedules.

Poster presenters should use Velcro tape to fix the poster onto the poster board. The tape will be available at the Conference Information Desk.
Please note that the Organisers will not be liable for any damage or loss to the posters.
Authors should acknowledge any conflicts of interest/source of funding in the oral or poster presentations.
Prizes will be awarded to the best poster presented.
5) ORAL PRESENTATION

The Scientific Review Committee will invite authors of selected papers to make an oral presentation at the Scientific Presentation session.
Prizes will be awarded to the best papers presented.
Information on oral presentation will be communicated to authors of selected paper.
6) DECLARATION
IMPORTANT NOTE TO PRESENTING AUTHOR
Submission of abstract herein therefore constitutes that the author understands and accepts of the following declaration and conditions:-

I understand and accept that authors should acknowledge any conflicts of interest/source of funding in the poster/oral presentation.
I declare that the information I have submitted herein is correct and accurate.
